These come around every so often but its always good to see who has loves what game.
Rules:
1) Doesnt matter when the game was made (From Pac Man to Counter Strike)
2) Can be from any platform (NES, PSX, Gameboy etc)

My 5
5: Goldeneye (n64) - My first shoot-em-up. Best map every created was the facility.
4: Football Manager (PC) - I spent hours and hours playing this. I remember in Season 2049 with Barcelona, I had 800 000 000 in the bank and all the best players. Game Over!
3: Grand Theft Auto (PC) - Nothing beats driving around listening to music and of course killing cops :) its the DT way
2: Zelda (SNES,GB,n64,GC,Wii) - More than 1 game I know so I might be cheating but great story and gameplay everytime
1: Secret of Mana for the SNES and Planetside for the PC. I cant decide between 2 games so I have to go with a tie. Secret of Mana was the game I grew up playing from when I was around 10 and I found myself going back when I was 14/15. It took around 72 hours to complete when rushing through and could easily take 100+ hours to finish on when playing normally. Ah Planetside, I think I have had just about every emotion going when playing that game. When you have the knowledge the game is simply mind blowing. I do not think anything will beat it even Planetside 2.

Not sure what order to but these in, but:
Renegade, Goldeneye, Planetside, Zelda OoT, and C&C:RA are probably the games that have captivated me the most of the years.

In no order:
Planetside, Counter-Strike, Final Fantasy 6 (aka FF3 on the SNES), and SMB3
My top game of all time without question is Earthbound on the SNES. This game just rocks. Its a jPRG set in "modern times" so you kill shit with bats and yo-yos. Nintendo could probably make a jillion dollars if they'd release this on the Wii's VC. It is also the source of Ness in SSB.

In no order:
Doom 2 (PC) - If you didn't play Doom 1 & 2 you can't call yourself a FPS lover.
Megal Gear Solid (PS1) - Amazing game in its day
Rainbow Six: Rouge Spear (PC) - So many hour spent on this... love the older RB6 games, none of this Vegas crap.
Command & Conquor: Red Alert (PC) - I still play it... but reguardless what I do I can't get it to run long on my WinXP without crashing
WorldWar II Online (PC) - Greatest MMO
I'd also like to give mention to:
Starwars Galaxies - first year or 2 it was the greatest, and still has features no MMO to date has touched
DeltaForce2 - 50 players per map in 1999! and it came with an awsome headset I used for years to follow
Counter-Strike - spent far too much playing this game
Planetside - First couple years were promising
Halflife - Epic single player game
TFC - My goodness, the thought of a teamwork game made me cream myself

Had a good think about this, somehow managed to narrow it down to five
5. Fallout 3 - Oh how this game has drained up time I should have been devoting to college work over the last year ha. I was sceptical at first how the Oblivion engine could port over to an FPS, but my god did they do a fantastic job. The most fun and memorable game single player game I've played in recent times.
4. GTA: Vice City - I remember falling in love with this game after just having playing 5 minutes of it... The first car you get into, Billy Jean pops on the radio, iconic moment. Most people think San Andreas ripped Vice City apart, but damn the core gameplay, the missions and the 1980s feeling you get from the game are breathtaking.
3. Sonic 3 & Knuckles - Sonic 3, what an amazing game. Pop it into the Sonic & Knuckles cart and you get the best platformer ever made. Mario blah blah, Im speaking from personal experience since I grew up with Sega.
2. Final Fantasy Vii - Six is known to have a better story, and ten better gameplay. But 7 still is my favourite... I love the characters, the story, the world, the amazing (for the time) cutscenes. Im replaying it right now and loving it.
1. Half Life 2 - More than a game to me, but an entity that spawned bazillions of mods/maps/models etc etc that still has a huge community backing today. Valve are still releasing games with the source engine, and why not? Its showing its age now sure, but its still as fresh and innovative as it was back in 2004.

Nice bump. I am too lazy to decide on "top 5" so I just picked a few of my favorites for each console. Which 5 are my favorite is quite debatable.
PC:
-Everquest 1 (my first MMORPG! EQ2 did everything better but it will never have that classic feel)
-PlanetSide
-Solitaire (I play it all the time =x)
-SWG CU/NGE (CU I only really played for a month but I remember hardcoring it and people wondering where I disappeared to. NGE gets a mention due to how bad I dominated it.)
Honorable Mentions: Diablo 2 (I loved it in 1999-2002 wasting thousands of hours in it but its shit now)
PS2:
-Tales of the Abyss (Tales series is huge in Japan but westerners tend to hate it due to its presentation. VERY anime-like.)
-Suikoden 5 (It is close to the most well-made JPRG ever, so much extra stuff that makes it great - its presentation is not for westerners which results in a lack of sales here. Insanely underrated.)
-Shadow of the Colossus
-Devil May Cry 3 (Though 1 is great and 2 is okay, 3 puts them both to shame)
-Kingdom Hearts 2 (1 is good too though)
-Monster Hunter
-We Love Katamari
Honorable Mentions: Dark Cloud series, Rogue Galaxy (if it didn't start off retarded), Shadow Hearts series, Persona 3 & 4

Honrable Mentions: Every game developed by Quintet for the SNES (though I haven't gotten around to some of them I would like to), Secret Of Mana (which may be higher if I replayed it with friends)
Gamecube:
-Crystal Chronicles (Everyone hates on this game but playing this with 3 friends is glorious fun)
-Super Smash Brothers Melee (puts the first to shame and the third is basically copy+paste of the second)
PS3:
-Bayonetta
-Demon's Souls
-Katamari Forever
-Valkyria Chronicles (okay, maybe it doesn't deserve a top spot due to how overpowered Scouts are.. which completely broke the game)
Honorable PS3 mentions: White Knight Chronicles (I kinda love what Level 5 do with console games but the grind at the end of this game is beyond stupid), Devil May Cry 4 (it is fun but its really DMC3 with pretty graphics plus the devil arm is cheesy as fuck), Folklore (okay, maybe I need to replay it but I recalled liking it a lot)
NDS (I actually hated the DS but some games managed to be fun):

Wii:
-Monster Hunter Tri

N64 sucked, YEAH I SAID IT.
Dreamcast wasn't bad (Power Stones I remember was fun). Never cared for any other Sega system.
Playstation had some games I would like to replay before listing them as favorites. (FF7, FF9, Chrono Cross, Legend of Dragoon, etc.)
Gameboy was crap but I recall million of hours in Pokemon Red, Yellow, and Blue.
Fuck XBOX/XBOX360. Faggots.
NES was quite horrid too for the record.
I am too tired to think of others. Yeah. Deal with it.

In light of this thread necro I feel I should spam it some more with a better list. Like Angel did.
Clarifications:
PC List:
C&C The First Decade: The first PC game I properly got into [aside from Lemmings and Dizzy] was C&C and for that I love this series and this game pack was one of the best deals ever to be invented.
Planetside: 'cause it was PS.
I don't know why MW1 was on the list, it was great but not epic so instead...
Tribes 2: Lost hours and hours to this game and it was the first game where I joined a clan and hardcored it to death.
System Shock 2: One of the best games ever in my mind, everything about SS2 was fantastic.
Half Life: Another game I played a hell load of, its great and you all know why.
Sega Mega Drive: The first console I ever owned.
Micro Machines: I used to love collecting the little cars and when the game came out I played it a lot, its where my addiction to racing games, even going so far as to but GT5 before a PS3, came from.
Sonic The Hedgehog 2: Sonic in his prime.
Can't recall the PS and PS2 era to well but some games:
Colin McRae: Love rally driving, loved this game.
Gran Turismo: The game series, next to PS, that I have most likely lost the most time too. I've played every incarnation of this series and 100% them, 'cept for GT2 which you couldn't.
Burnout: Yet another driving game [I didn't see much point in shooters on consoles as I had a PC so most of the games I had for consoles were either exclusives or driving games] this gaming series has also taken a lot of my life away, Burnout 2 especially as at one point in life I could hold unlimited Burnout runs on every track. Needless to say my friends never liked racing against me.
Ico + SotC: Great games with a unique take on gameplay.
Grand Theft Auto: Love this series, I remember sneaking GTA3 out of the cupboard my mum hid it in to play it late night before Christmas and being amazed at the new world that was opened up to me.
Honourable mentions:
Supreme Commander/Total Annihilation: I basically grew up playing RTS games and TA was a defining RTS game, lost hours to DLing mods and playing against my mates and still do with SC1 and 2.
Battlefield series: I've played every single BF game and every expansion pack and after tribes, this was the next game[s] where I joined a clan and hardcore'd it to death to get good.
Zone Of The Enders1/2: Fantastic mech series that was criminally overlooked. First game was very short and restricting but had such a unique experience I couldn't put it down. The second game fixed a lot of issues from the first and was an epic tale, I still replay 2 from time to time.

SC2 basically just made the action come faster and kept it flowing. Me and my mates still prefer SC1 as when you finally get to the big fights they're about 4 times more epic, but SC2 is ok for what it is.

Hehehehe. I used to have to hide GTA3 from my parents, I knew my dad would kill me if he knew what it was. Whenever they walked into the room I would just press pause and mess around with the settings lol.
One game I forgot that is easily one of my favorites...Heavy Rain. If you've never played it give it a try.

Beyond those four, it's very hard to choose just one. Some great multiplayer games I've enjoyed would be Counterstrike, Halo (any of them), C&C Red Alert 2, battlefield 2... COD MW and MW2 were also my steez for a liiiiitle while. TF2...
some great single player games ive enjoyed would be Bioshock, Sonic & Tails for Sega Genesis, Sonic Adventure 1 and 2 for dreamcast, oblivion, sw kotor, many of the CODs had great single player campaigns... damn its been a while since ive played a single player game all the way through. OH portal 2 was fucking dope. I guess that's pretty recent.
However--playing PS, whenever I was in a teamwork based outfit (PRG back in the day with Dart, us obviously) was the best time ive ever had with games. Everything since then has basically been second rate. These days I only really play super smash or NBA with friends. I know planetside was essentially a piece of shit game but man we could spend days playing that shit way back when.
C&C Renegade was probably my fav game of all time, did any of you guys ever play on the clanwars.cc leagues? we used to run as +BR+ won 5 or 6 consecutives cups back in the day with that game, i do belive planetside took many ideas from that game, stealth suits and tank designs, 3rd person etc..
